Instructions and Applications for a Replacement Certificate due to Hurricane Florence

To help those people who lost their homes and belongings during Hurricane Florence or because of flooding related to the storm, Governor Roy Cooper issued Executive Order 67 on October 3, 2018. This Executive Order suspends the collection of fees associated with processing and issuing replacement vital records obtained through the N.C. Department of Health and Human Services’ Vital Records Office, while North Carolina is under a declaration of emergency.

For those requesting a replacement of a vital record under the authority of Executive Order 67, please complete the appropriate form(s) below.

NOTE: Identification of the individual requesting a certificate is required. For a list of acceptable IDs see What forms of IDs may I use to obtain a certificate? on our FAQs page. Requests that do not include proper identification will be returned.

What we have:

Only certificates for North Carolina registered vital events, including the following:

  • Birth certificates are available starting in 1913.
  • Death certificates are available starting in 1930.
  • Marriage certificates are available starting in 1962.
  • Divorce records are available starting in 1958.
  • Fetal Death Reports are available starting in 2001.

2a. Choose Standard Service: Mail or Drop Off Form

You can also drop off the form at the Vital Records Office in Raleigh between 8:00 a.m. and 3:00 p.m., Monday–Friday, except on state government holidays off-site link. Directions to our office.

If no record is found, you will be sent a letter.

 

 

2b. Choose Faster Service

There are two ways to get faster service.

  1. Delivery through the Mail: Check the box for expedited service on the application form.
  2. Send via U.S. mail to:

  3. In-Person: You can receive same-day, walk-in service in the Vital Records Office in Raleigh. Walk-in service is available between 8:00 a.m. and 3:00 p.m., Monday–Friday, except on state government holidays "off-site Directions to our office.
